ANNE HUTCHINSON PTA EVENT: END OF YEAR 2026
WAIVER AND RELEASE FROM LIABILITY
PARTICIPANT'S GENERAL RELEASE
Event: Anne Hutchinson End of Year Festival, “ALL STAR SUMMER” (the “Event”)
Location: Anne Hutchinson School, 60 Mill Road, Eastchester, NY 10709 (the “School”)
Dates: Monday, June 15, 2026, and
Times: 5:00 PM and 8:00 PM (together the “Event Schedule”)
By signing my name below and in consideration of one or more of the following activities to be presented by the Anne Hutchinson PTA as part of the Event held at the School during the outlined Event Schedule:
Name of Activity: Bungee Trampoline, Obstacle Course
Description of Activity: jumping, playing, running, climbing on designated inflatable structure or trampolines
Vendor Name: NJ Party Time Rentals
Name of Activity: Whirly Bird Ride
Description of Activity: riding on designated spinning ride
Vendor Name: AIO Entertainment
All above listed activities, as well as additional ancillary activities, shall together constitute (the "Event Activities").
I attest that I am physically fit and capable of participating in the Event Activities at the Event to be held at the School during the Event Schedule listed above. I also understand the nature of the Activities that I will be involved in, and I will strictly adhere to any age, height, rules, restrictions and/or code of conduct for such Event Activities, including the use of any required Equipment. I understand the nature of the Event Activities that I will be involved in and acknowledge that it is an extreme test of a person’s physical and mental limits and carries with it the potential for death, serious injury, or property loss. I hereby assume the risks of participating in any activity associated with the Event Activities.
I hereby take the following action for myself, my executors, administrators, heirs, next of kin, successors and assigns:
- I waive, release, and discharge from all liability, claims, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or any of my property, arising from or in connection with my participation in the Event Activities, the following persons or entities: The Eastchester School District (the “District”), The Anne Hutchinson School (the “School”), The Anne Hutchinson PTA (the “AH PTA”), NJ Party Time Rentals, and AIO Entertainment, (together “the Vendors”) and any related or affiliated school entities and companies, partners and their respective officers, agents, employees, licensees, consultants, independent contractors of any of the above.
- I agree not to sue any of the persons or entities mentioned above for any of the claims or liabilities that I have waived, released or discharged herein; and
- I indemnify and hold harmless the persons or entities mentioned above from any claims made or liabilities assessed against them a result of my actions, including claims that are known and unknown, foreseen and unforeseen, future or contingent, or that arise out of the negligence of the persons or entities mentioned above. Being fully aware of the risks inherent in participating in the Event Activities, I hereby elect voluntarily to engage in such activities, follow any directions given by staff and the persons or entities mentioned above and assume all risk of loss or injury that I may sustain in connection with the Event Activities.
In signing this General Release, I hereby acknowledge that I have read the foregoing release and fully understand that this is a release of liability that will legally prevent me or any other person from filing suit or making any other legal claims. I nevertheless enter into this agreement freely and voluntarily and agree that it is binding upon me, my heirs, assigns and legal representatives.
I HEREBY AFFIRM THAT I AM EIGHTEEN (18) YEARS OR OLDER, AM PHYSICALLY FIT TO PARTICIPATE IN THE ACTIVITIES AND I HAVE READ THIS DOCUMENT, AND I UNDERSTAND ITS CONTENTS.

I AM UNDER THE AGE OF EIGHTEEN (18) YEARS. MY PARENT/GUARDIAN HAS READ AND COMPLETED THE SECTION BELOW.
(If applicant is under 18 years of age, a parent or guardian must execute, in addition to the foregoing Waiver and Release, the following, for and on behalf of the Minor. All applicants must be four years of age or older to participate in the Events activities.).

I hereby state that the Minor(s) named herein are four years of age or older. I hereby bind myself, the minor and all other assigns to the terms of the Waiver and Release. I represent that I have legal capacity and authority to act for and on behalf of the Minor named herein, and I agree to indemnify and hold harmless the persons or entities mentioned above for any claims or liabilities assessed against them as a result of any insufficiency of my legal capacity or authority to act for and on behalf of the minor in the execution of the Waiver and Release.
